Bring 5 cups whole milk to a simmer in a 4-qt. heavy saucepan. Transfer 1 cup hot milk to a blender and set aside.

Whisk ¾ cup (105 g) malted milk powder and ⅓ cup (packed; 71 g) dark brown sugar into remaining hot milk until dissolved. Remove from heat and stir in 2 tsp. vanilla extract and ¼ cup light rum (if using).

Blend milk in blender at high speed until foamy, about ⏱️ 1 minute. Divide malted milk among 4 mugs, then top with foamy milk and sprinkle with freshly grated nutmeg.
