Heat a 12-inch cast-iron skillet or heavy bottomed pan over medium heat; add the butter. Once the butter is melted and starts to bubble, add the onions. Cook over medium heat, stirring frequently, for ⏱️ 2 minutes. Reduce heat to low and cook, stirring occasionally, until onions are deep golden brown, 45 to ⏱️ 50 minutes. Add the balsamic vinegar and stir to combine; cook 2 more minutes.

Beat the cream cheese in a medium mixing bowl with an electric mixer on medium until smooth. Add the sour cream, mayonnaise, Worcestershire sauce, garlic, salt, pepper, and reserved caramelized onions, stirring until combined.
